If your employer or health plan asked you to use this Extracon app to log your health data and earn incentives, then you’re in the right place. You will track your weight, blood pressure or other health data, and also use the app to take a picture that proves your data is accurate.

You can only use this App if your employer or health plan is sponsoring a wellness program that uses this App. You will need to log in to use this App. If you don’t have a Username and Password yet, go to www.extracon.com and click “Register” before you download this App.
